The safety and wellbeing of our students is the top priority of Davidson Charter Academy. We are committed to working with our student’s families to ensure that healthcare needs are met. With that in mind, here are a few things to remember as we start the new school year. Please use this checklist to help ensure safe medication administration. Remember that this applies to any medication including cough drops, eye and ear drops etc. used to treat symptoms or medical diagnosis your child may have during the school day.
When bringing medication the nurse’s office, please provide the school with each different medication in a separate appropriately labeled container.
Prescription Medication || Must be placed in a prescription container indicating the:

Child’s name

The name of the medication

The unit of dosage to be given

The number of dosage units

The time the medication is to be given

How it is to be administered (It is recommended the parent ask the pharmacist to provide two properly labeled containers – one for home and one for school).
Over the Counter Medication || Must be placed in the original container labeled with the:

Child’s name
-
The name of the medication

The dosage to be given

The time of administration

The method of administration
It is important to provide to the school the “Parent’s Request and Physician’s Information for Administration of Medication in the School” form and ensure the form is completed, signed by the health care provider and parent/guardian, and returned to the school. The health care provider may use another format (letter, computer printout, etc.) to authorize the administration of medication as long as all information requested in the DCA Medication form is provided. If you do have a Medical Authorization form from a Healthcare Provider or a school system other than Davidson Charter Academy, you will need to complete the Use of MAR Permission Form.

The non- prescription medication section (A) can be completed by the parent or guardian. It is important to fill out this section as completely as possible to ensure safe administration of the medication.

The prescription medication section (B) needs to be completed by your Healthcare Provider with the parent/guardian signature and phone number where indicated.

The Medical Release of Information section needs to be completed by the parent or guardian. This is important to allow DCA staff to request information from your Healthcare Provider if needed.

Provide new containers with appropriate labeling each time the child’s medication changes, and to remove from the school grounds any medications discontinued by the child’s health care professional.

Remove all medications from school premises at the end of each school year. If not removed, the school will dispose of all medications in its possession two (2) weeks after the last day of school for students. (Whenever medications are removed from Davidson Charter Academy, the Release of Student Medication form must be completed by staff and the student’s parent or guardian.)

Parents/guardians are encouraged to bring medications directly to the school office and avoid sending the medications to the school via the student.

The authorization for administration of medication will be valid for only one (1) school year.

No medication (either prescription or non-prescription) will be administered without written order from the health care provider and the signed consent of the parent/guardian.

If the completed form is not received and it is necessary for the medication to be given, the parent may come to the school and administer the medication.
Some medical conditions require an Individual/ Emergency Action plan. These conditions include, but are not limited to:

Anaphylaxis

Asthma
-
Diabetes
-
Seizures
If your child has any of these conditions, Individual/ Emergency Action plans can be downloaded from the 'Forms' page. These need to be completed by your Healthcare provider and returned as soon as possible and are good for one (1) school year.
FAQs
Can I put multiple medications on a Medication Authorization Form?
Yes, as long as it is a non prescription and prescription on the same form. However, multiple prescription and non prescription require multiple forms.
How long does my student need to stay home for lice?
Your child may return to school when he/she has been cleared by a doctor, health department, or school nurse. Written documentation must be provided on his/her return to school.
Do cough drops require a Medical Authorization Form?
Yes, anything given to treat symptoms requires a medication authorization form. This also includes eye and ear drops.
How long does my student need to stay home if they have a fever?
Your student should remain home until their temperature is below 100.4 for 24 hours without taking a fever-reducing medication and demonstrates no other symptoms.
How long does my student need to stay home if they are vomiting?
Your student should remain home until 24 hours after the last episode of vomiting.
How long does my student need to stay home if they are having diarrhea?
Students should stay home if they have more than one occurrence of diarrhea.
How long does my student need to stay home if they are suspected of having Pink Eye?
Any suspected case of pink eye must be examined by a Healthcare Provider. The student's ability to return to class is based on the Healthcare Provider's findings. Bacterial conjunctivitis; after 24 hours on antibiotics. Viral conjunctivitis; with a note from the doctor stating child is no longer contagious.
